This page explains what happens to the information you share through wholesmilewellness.com — the opening list, the parent guide sign-up, and our contact and referral forms.

Please don’t include health information

The forms on this website, including the opening list and the contact form, are for marketing and scheduling purposes only. Please do not include your child’s health history, symptoms, or other medical details in any website form. Once you become a patient, your health information is collected and protected separately under HIPAA, and we will talk with you about it privately, by phone or in the office.

How we use it

We use the information you submit to respond to your message, to send the parent guide you requested, and, if you join our opening list, to email you when consultations open along with occasional updates before then. We do not use it to make any scheduling or medical decision on your behalf, and joining a list or submitting a form does not make you a patient.

Email and unsubscribing

We only send email to addresses that are provided to us through a form on this site, and only for the purpose described when you signed up. You can unsubscribe at any time using the link in any email we send, or by emailing us directly.

We don’t sell your information

We do not sell or rent the information you share with us to any third party.
